only insurance I accept is Mayo Clinic employee insurance. I am no
longer taking new Medicare patients. For other types of insurance, I am
out-of-network. I will give you a "super-bill" with the codes necessary
for you to submit to your insurance company if they offer partial
reimbursement for out-of-network providers.
